I like so much and it's very pretty. I'm interested especially in the Find Treatment options.
I've used in my web the Amelia Forms 2.0 but it doesn't show like your sample.
I added images and text description but neither presents like this option.
Can you tell me how can I do it like to present like as Find Treatment options??
Is it necessary another license or codification especial??

This is the old 1.0 search booking form that will be depreciated and there is no new 2.0 search form like there is with all other forms since this for was used I less the 3% of our users. There is a new 2.0 catalog booking form we can advise to use. Also all new features that we implement are not available in old booking forms that are not maintained anymore, so we advise you to browse through and use only new 2.0 booking forms.
