I have an issue that has suddenly arisen for no reason, where Amelia blocks either do not appear in the Add Blocks section, contain an error (Your site does not include support for the " "...) and occasionally do not appear when previewing the page (blank space or no space at all as if the block was never added). I have tried reinstalling Amelia and Gutenberg, clearing cache and changing theme, none of which work. I've read it might have something to do with existing plugins, but right now, Amelia is the only non WordPress plugin installed...
I hope I can get this fixed because it is driving me crazy!
I just wanted to double-check—are you currently using the Lite version of Amelia? If so, please note that the Lite version has some limitations, including restricted access to certain booking forms. If you have the full version, all Amelia shortcodes should be available in the builder selection.
Based on the screenshot you shared, it looks like the issue may be related to the website itself. You could try this workaround: open a new page, add the "shortcode" module, then copy and paste the same shortcode there, and publish the page. This is an alternative way to add the booking form to your website.

It could be a conflict with third-party plugins. In most cases, when the booking form isn’t visible on the frontend, it’s due to a conflict with a caching plugin. If you have a caching plugin installed, please try disabling it temporarily and test again.
Also, just a quick note—if you're using the Lite version, you can only use these two booking forms:
