In that case this could be a conflict with your theme. Did you try any other theme? Amelia is designed in such a way that it should have no issues with most of the themes but since there are millions of them we can not guarantee that. In that case most likely you would need to use some other theme.

Could you kindly send us the temporary admin credentials and specify exactly where the theme is not working? For example, are you unable to add the shortcode, or is the form not loading? Please provide more details, and we will gladly forward the issue to our Dev team for further investigation. Make sure that you send us this in a private response.

We just got the feedback from our colleagues and they have told us that they have tested it and can confirmed that the form isn't loading the way you added it. We also moved it to different parts of the theme, but without success, as Amelia's script is not being loaded on the page.

We either need FTP access to debug this and try to find a solution, or you can zip your custom theme and send it to us so we can test it locally.

We just got the feedback from our colleagues and they have told us that it seems that you didn’t call wp_footer() in your custom theme, as we couldn't see any scripts in the footer.
