I have a very simple requirement - book an appointment for an estimate to be done at the customer site. There is only one SErvice (estimating) and one Employee, so those fields are not input fields. I have gotten the form the way we want with a small exception. Here is what we take in at the "Your Information" time in the attachment.

I need to set the phone number and email address to required. I found the global setting for Email required (I think) but it's not working. When I go into the Customize Forms, it comes up with a very complex form with many steps that I don't want to show. 

How do I make just these 2 fields required on the form that I see when I access the page?
