An issue with email notifications could be caused by several reasons:
Your hosting provider is blocking all 3rd party settings and only allowing their SMTP (or other) settings.
Your website has a PHP version of less than 7.
The credentials are not correct.
There's something else blocking the notifications from being sent.

Here is my feedback regarding the points you mentioned:
My hosting provider (Infomaniak) does not block any third-party settings. When I create my user account in WordPress via Amelia, the emails are correctly delivered to [email protected], so there is no blocking at the WordPress level.
The PHP version on my server is 8.1.
The credentials are correct, as emails are successfully delivered when the employee uses a regular email address instead of an alias.
Indeed, there must be something else blocking the email.
My tests are sent from and to the same domain.
I can give you access to test and confirm what I’m describing.
I realized that you tried the workaround for sending emails to the same employee email addresses. This is possible, but it only works for Gmail addresses. You can use the same email address by adding a "+number" before the "@" symbol. For example, [email protected], [email protected], [email protected], etc. will be treated as different addresses when saving, but all emails will be forwarded to the primary [email protected] address.

We do not interfere with any data or anything else except for the plugin (in case that’s a production version of the site), and of course, we do not provide login data to third parties.
You can write credentials here just check PRIVATE Reply so nobody can see them except us.
The issue here is the email configuration is configured to PHP mail. This is not recommended since emails ends up as spam or they are not delivered at all. Instead, we recommend SMTP configuration, which you can find on this link.

I recently created two employee email aliases, but they did not receive the appointment emails as expected.
The original email where they should receive all emails is: [email protected]
I tested by sending a sample email to both [email protected] and [email protected], and it worked perfectly.
However, the appointment emails are not being delivered to these aliases.
I also tried using the following alias formats, but they didn't work either:
