Custom fields in Amelia are connected to appointments rather than customer profiles, so they will appear on the Amelia > Appointments page instead of the Customer profile. To collect medical history forms from customers, you can:

1. Add custom fields for the medical history form to the appointment booking process.
2. Once customers fill in those fields, the information will be available in the appointments section for the respective appointment.
3. Doctors (employees) can then view these fields by going to the appointments page in the Amelia dashboard.

Unfortunately, as of now, there is no built-in feature to display custom fields directly on the customer profile, but they can be accessed via the appointments page as mentioned.
