Thanks for reaching out to us. There is no need to reinstall the Amelia, the issue here is that you are not editing the correct version of the booking form. On the first screenshot, is the new booking form, on the second one is the old booking form. Therefore, you want to make sure that you are editing and previewing the same booking form.

Let me try to explain a bit more. As already noticed, you are editing a different booking form. In the Customize section, you have the option to choose the booking form according to your needs. 

For example, you want to choose the Step by Step Booking Form 2.0 and then when creating a new page, you want to make sure that this new booking form is displayed. 

All customization needs to be done here

But, when creating a new page, you want to select Step by Step v2 module 

In this way, you are displaying the same booking form that you edited previously. 

It's the same way for the old V1 booking form, but we recommend using the new booking form since they are updated and the old v1 will be deprecated in the near future.

You probably mean the booking form? The plugin should be activated already in Plugin section.
