Is there any way to have numerous customers with one email account? We have a number of children or elderly patients who are booked in by their parents/children but we cannot complete customer accounts as they don't have email addresses and won't accept an existing email address.

Yes, of course. In settings role settings just turn this this option and make sure that you purge the cache
That should be it. If you test this make sure that you test from incognito mode to avoid creating conflict of roles.
