We can't replicate this locally with just WooCommerce installed:
The description has been copied from your website, but since there's no custom field on my installation, I see the placeholder.
Appointment Date and Start Time are, however, there, as you can see in the first screenshot.
Since you have quite a few WooCommerce extensions, my advice would be to disable them and then test again. If the cart populates with the data you entered in Amelia, then it's one of the WooCommerce extensions causing the issue.
We confirm that the integration works with WooCommerce, but we can't guarantee that it works with all extensions available for it. If you isolate the extension in question, I will be able to ask our developers to check if there's anything we can do to help you resolve the issue, but I can't promise anything.
In case you want to modify it further, it's in settings, under "Payment Settings" - when you scroll all the way down, just expand the highlighted section and edit in HTML mode:
WooCommerce made some changes in the cart, so I had to add "A" in the first row, like this:
div.shop_table.woocommerce-checkout-review-order-table > div > div > div.product-info > div > dl > dd > p:nth-child(1) {
opacity: 0 !important;
}
To avoid seeing that "A". Just remember that this needs to remain in the description in Amelia as the first line in order for the description to appear correctly.
Custom code needs to be applied to the page where the booking form is. Depending on what you're using (Gutenberg blocks, or some page builder), adding the CSS or JS can be done in a few different ways.
If you need help with adding custom CSS to the page, please take a look at this article.

Hi there,
understand it might due to other plugin that affect the functionality,
we have done testing with other plugin,
it seems like it doesn't work together with one of the point system plugin, Point and Rewards for Woo-Commerce
Can you look into this?
We have updated the plugin, it is somehow showing the booking date & time, but the alignments look weird
Thanks

Hi again Dickson.
This is how you added the placeholders in the WooCommerce description in Amelia Settings/Payments:
Once you edit this and separate the placeholders, it should reflect on the front-end.

hi there, couldn't where to modify the code, can you login to the website backend and update for us?
Of course, Dickson, I did it for you.
In case you want to modify it further, it's in settings, under "Payment Settings" - when you scroll all the way down, just expand the highlighted section and edit in HTML mode:

I'll ask you to post this CSS on the cart page:

Also, update the Checkout Page with this CSS:
