We are trying to customize and put our own enrollment form into the calendar label so that when they are in the final steps to complete the appointment they answers these questions we have came up with and we want to be able to print the for out after they complete it, of you could please help us out asap
The only things that can be added are custom fields, but they can only be visible on the "Your information" step.
Later, those custom fields that the customers have filled can be added to the notification by using the %custom_field_x% (x being the ID of the custom field) placeholder.

Can we not add a form into the calendar that they fill out?
