Can you please provide more information about the way you want to customize the booking form? I don't see any picture attached, and from the ticket info, the only link attached is the demo of another plugin whose page is empty at the moment.

We've implemented options to completely customize the booking form, from changing colors to reordering selections to changing the button types, so there is really lots of flexibility. If you want to try all those options on the admin side, our Fitness and Yoga demo has this option, just click on the Back-end Demo and log in. Just for you to know, the demo refresheses each 15th minute of an hour so the changes will be lost.

It is possible to put selections in that order as you can see from the picture below:
However, in Amelia, as you can see from the picture Location, Service, and Employee selections are on the same step, so the display style is not the same.
Services are displayed with name and price (if you have prices), locations with names, and employees are shown with pictures and more info (if you add the description), you can see the example below:
