Can you please check do you have the wpDataTables Plugin 

and Forminator Plugin both installed and active on your WordPress?

If you do not have one of them, you will not be able to activate the integration.

you have wpDataTables, and the integration, but you do not have the Forminator Forms installed.

As this alert on the top says, 

you need to install the Forminator Forms Plugin.

Please install that, and after that you should be able to use the integration.

What you downloaded is not the Forminator, that is our integration add-on,

the full name is :  "wpDataTables integration for Forminator Forms".

You can see that on the download Page, and also check in your Plugin List as the name.

I realise that on our Add-Ons Page it is just labeled as "Forminator Forms" ,

 i will speak with our Management to perhaps change this to add something like "integration" just to clarify more for the users to avoid confusion.

But to summarize, this is not the actual "Forminator" Plugin, 

that is rather just our add-on which provides the integration between wpDataTables and Forminator Plugin.

Now, you can just go to "Add new Plugin" , type in "Forminator", and download, then install and activate it,

after that it should work.

Or you can download it from their WP Page of the Forminator Plugin, it is here.
