I know i can create a table based on form entries, but what about the other way around?
I mean, i want to create a form with several selective options (checkboxes) and i'd like to pull those options from a table or database dynamically. And it would be great to also be able to conditionally display those options too.

We do not have any Form Plugin of our own yet.
We have a Plan to develop our own Form Plugin and to integrate it with wpDataTables,
but i don't have any ETA when that might be finished.
Even when we finish our own Form Plugin, i am not sure if something like that use case will be possible,
even though it is a very nice idea - to get the options for a selectbox/checkbox for a Form be pulled from a Database Table.

You can check with the support of some Form Plugins if their Forms can be populated for selectbox/checkbox in that way from any SQL Table and see if that is possible.
For example, if you create a Manual Table from our Plugin ( that is only available in our Premium wpDataTables version),
this will create a new 'stand alone' Table in your SQL database, with its own set of columns and rows;
So then you will be able to pull data from our Manual Table easily with SQL Queries.

If you get any Form Plugin working to pull data from a Database Table like that,
then yes - you can make a Manual Table with our Plugin, set any possible values you need,
and then pull them in your Form,
but as i said - this is only in Theory, and as we do not develop any Form Plugins, we are not able to confirm if that is currently possible.
And i just wanted to point out that you can try wpDataTables before purchasing
