Under general settings we have this ticked
Does this have to be ticked for customers to be amelia to register for amelia accounts to book appointments?

This setting is not related to Amelia, but is referring to "anyone can register" on your WordPress site.
If you're using a membership plugin that blocks pages where Amelia forms are for non-logged-in users, then I suppose this option needs to be enabled, but if you haven't imposed any similar limitations, it doesn't have to be.
