In this custom site the form on home page is connect with a database. The field "club" is a database table. When users select the club and fill the other fields, the form send to a user an email with the data input plus the information (that is located in a same database but in an another table) of the club.
Is possibile replicate this function with yours plugin?

I can see the Form on the page, I guess it is this one, where a user can input their "Club" of choice, their Name, phone, Email and hit this "submit" button?
If you just needed this functionality as a Form plugin,
and to output a table from these Form submissions, in that case we would have an easy solution.
We have several built-in integrations with form plugins.
Then you could make a table in our plugin that will be connected to the Form, and it can output user's submissions ( along with any other fields you place as needed);

But for the form to send an Email to the user, and for the Email to additionally pull data from another table;
At this time, we don't have any built-in solution to achieve this, but you can suggest it to our developers - they will do their best to make a solution in the future.
to see if someone may be already suggested this feature. If you can't see it, feel free to add your suggestion there, and as more people vote, the feature will move higher on the priority list.
