Formidable form with 3 columns (reason, count, pct) and these are repeated for multiple rows. User enters a reason and assesses a count number. That then gives a pct based on all entries. The user will repeat this process up to 15 times to create a list of reasons and assessment count for each reason with a corresponding pct of each assessment. I need to bar chart the reason (x-axis label) with the pct of each reason. I have attached the actual form created and a working view of the process from excel. Any help would be greatly appreciated.

If they are,
I am sorry to disappoint you, but repeater fields from any Forms plugin cannot be correctly added in wpDataTables, so you wouldn't be able to use them at this time.
Each repeater field would be saved as a new/separate Form when it gets passed to our tables.
Right now, this is a limitation we have - but our developers will do their best to make an improvement in the future.
