Currently, we have a bug if you place both old and new Form together on same page - The devs are working on a fix, i can't promise an ETA - but for the time being, please avoid setting both Forms on same page,
you can set one page for Old Form, and on another you can place the New Form.

If the issue is that for logged-in users the site looks different then for non logged in users;
The issue might be in your site security settings.
They're either configured in a Security plugin (if you have any), or they're on your server's side.
The best option would be to reach out to your hosting provider, send them the link of the booking page and tell them exactly what you told us - you can see the page when you're logged in as an admin, but you can't see it from an incognito window (or when you're logged out).
They should be able to lift the block, so admin-ajax calls are sent for non-logged-in users also.
Let us know how it goes, please, that should resolve the issue,

Additionally, i noticed on one screenshot you sent me, the error said :

- This CORS error occurs when your site is trying to load files from a different domain than your WordPress domain, so your provider will need to help you adjust your CORS settings to make them less restrictive and allow these files to be accessed.
Please provide me a temporary WP-admin (administrator) user for your site where this happens, so we could log in and take a look ‘from the inside’ as that’s the most efficient way to see and resolve the issue.
We do not interfere with any data or anything else except for the plugin (in case that’s a production version of the site), and of course, we do not provide login data to third parties.
You can write credentials here just check PRIVATE Reply so nobody can see them except us.
