So this text appears as a hyperlink that opens a new tab (target="_blank") and takes the customers to your Terms & Conditions without them leaving the booking form. So the custom field on the back-end would look like this:
As you can see, we made this radio button a mandatory field, so no one can book an appointment without checking the "I agree" button.

Yes. The amount of time slots available for booking during a day are directly linked to how many employees you assign to a Service, and how many work hours you give them ( you can also make custom work hour "blocks", breaks, etc) ,
along with the duration of the service.
You can customise the schedule, please check out more about that here.
- Conditional fields or notices (which appear or disappear in the form based on the service or based on the number of people who wish to book);
Currently we don't have any conditional logic in Amelia
Our devs will work on adding it, but i can't say an ETA on it.

That is only available if you use our Package of Services feature.
Packages in the Amelia Booking plugin are made to give you the option to create a package of multiple appointments from the same or different services and give a discount to the total price to attract your customers more.
Please note:
Packages feature is a part of our Pro and Developer licenses and isn’t supported in the Basic license.
At the moment, Events cannot be used with Packages, only Appointments (Services).

Yes. You can set up how you want the default booking status to be, it can be accepted as soon as customer makes a booking.
Or you can set Pending, and then later for someone to approve it from back-end.

Creating a form like on the example is absolutely doable with Amelia.
The date and time selection is pretty standard;
the name, surname and email , as well as phone number are built-in regular Form fields;
you can also add as many Custom Fields as you like (more details on all the custom field types available here).
Regarding the terms and conditions, we have a workaround like this :
To add a link to Terms & Conditions, you can use "Checkbox" or "Radio" custom fields. In the custom field you need to add an anchor tag like this:
