When I go to manually add an attendee in the back end of an event the fields for phone number and email are missing. My new custom fields are shown but those two default ones are missing.

There is nothing "missing" this how Amelia is designed it only shows these fields when you try to add attendee
You can try to add attendees for now from the front end you can book for them from the incognito window.
I'll kindly ask you to add it as a feature suggestion on this page.
Features are pushed up on our "to-do" list when there are a lot of customers requesting those features, so having your vote as a customer can be beneficial to this feature being developed sooner.
