Of course, if you set the "Default Time Slot Step" value to 5 minutes. You will be able to choose between 5-minute duration, 30-minute duration, and 1-hour duration.

Unfortunately, it is not possible to set the default time slot step for each of the services as it is defined globally. 

For the purpose of getting additional information from the customer, you could create custom fields where you can specify what you need from the customer to enter when they are booking an appointment.

Firstly, I would like to sincerely apologize for the delayed response as we have been experiencing an unusually high number of tickets. I am sorry that it has taken longer than usual to respond to your concern and your patience is highly appreciated.

You could change the labels of those fields, but the issue with that approach is that those fields will be saved in the database as email, last name, and first name, thus it would be suggested to add custom fields for those additional fields you want to add to the booking form.
