You can remove the Employee and Location fields for step 4 of the Step-by-step booking form by choosing customize option in Amelia and then by clicking on the 4 step and on edit option for the fields you want to remove just like on the picture.
Every form can be customized we suggest that you explore it and play around with it a little more.

Custom code needs to be applied to the page where the booking form is. Depending on what you're using (Gutenberg blocks, or some page builder), adding the CSS or JS can be done in a few different ways.

Unfortunately, that line is not possible to remove because it's not in the block that can be removed or modified. It is built-in.
This will be added in one of our future updates, but until then you can hide it by adding the following CSS to the page:
