    Hello, I would like to be able to put a form for each attendee of the event in case there is more than one attendee. Is that possible?

    Thank you for reaching out to us.

    I'm afraid, at the moment Amelia doesn't have such a built-in feature.

    However, you can collect information about additional attendees through Custom fields. Please note that in this case new Customers won't be added to Amelia, and these additional people wont be able to receive automatic notifications because this information will be saved as a note to the Appointment or Event. 

    Also, you can suggest that as a feature request here, or check if this request was already submitted and vote for it.
