Is it possible for the customer when booking his appointment to leave a personal note (simple text) for the employee ?

Yes, it can be achieved with Custom fields.
You can create a text or text area Custom field on the Amelia -> Customize -> Custom fields page and assign it to the Services or Events that should have this note.
