There isn't such possibility in Amelia with its current built-in features, unfortunately, and there isn't an integration with such a tool available for Amelia. Custom field content will change in each email sent depending on what customer entered, but the content of the whole template will be the same.
You can suggest that as a feature request here, or check if there is such request there and vote for it.
Is there any wp hook available to handle before email sending event when an appointment is approved.
For example, the idea is to send a different email template regarding a selection from the custom fields in the form.
