It is weird that you said, because I have seven custom fields now, I can see all of them on the back end except the custom field "Attachment", why I can see the other custom fields and I cannot see the custom field "Attachment" on the back end?
Regarding this topic, like you said I see the custom field on the front end (appointment page), but when the employee want to modify the appointment to approved status or any appointment data I do not see the ability of upload a new file using the custom field "Attachment, any reason? I want to upload a picture file when the job is done, for example, My employee has a client's appointment, so he goes its place and he makes the job, take a picture of the job done and he needs to upload that file (picture), he signed in on the employee panel and the system does not show an option to upload the new file, why? We are doing reference to the front end.
Saddly all my last questions have no solution, I see many limitations now.
One more question:
When I making a client'sappointment using the front end (appoitment page), how can i choose the client that is already on the customer section, I want to create a client's appointment for a client already have on my database, how can i choose that client, I have to press any key or something like that to see the clients catalog or what I have to do?

1. I'm afraid, at the moment only Customers can upload attachments. Neither Employees nor Admin/Manager can do it. I can ask our development team on your behalf if they are gonna change this logic any time soon.
2. I'm not sure I understood the scenario.
If you mean booking on the front-end through the booking form, then there's no such an option. More than that: you should not book appointments for your customers being logged in as Admin or Employee since it can cause mixed user permissions.
If by "using the front end" you mean the front-end Employee Panel, then you can just start typing Client's name in the corresponding field and it will pop up.
In my case I work with warraties companies they give the appointments already scheduled I just need to do it with my employees, so that means I need to register those appoitments myself or my employees on Amelia, specially myself from the back end and my employees from the front end (Employee Panel), supposed they have to had the same functionality in both sections, and both has to work the same way. But I see Amelia works in different way as back end and front end. For me that is not making sense, i think for nobody.

Unfortunately, I am not a developer myself and I can't explain why our developers made this decision from a technical point of view, but there is some limitation that didn't allow to implement adding files from the back-end.
Also, the Employee panel is created for Employees, and this role has limited permissions in Amelia. Admin should access Amelia from the dashboard, that's how Admins administrate sites, so there's no point to add a separate panel for them.
I forwarded your case to our developers, so I hope it will help to speed up the process of implementing a new feature for attachments from the back-end. It's the only thing I can do right now.
