Some form is disabled and can't put the values to them.
I noticed that it happens when I'm logging in with amelia employee or amelia manager account. And some clients let us know that the same thing is happening on their computer.
I took a look into the code, and it has "is_disabled" class to the wrapper div of the single input, also there's "disabled="disabled"" attribute in the input tag.

thank you for reaching out to us! I apologize for the later response, due to the Easter holidays we've had.
If the fields are pre filled with customer information, that happens when they are logged into their WordPress account and are making an appointment. They need to log out if they want to put different information into the fields. Also, you can go to Amelia > Settings > Roles > Customer > turn off Check for existing email if you don't want your Amelia customers to be limited to the data they entered during their first booking.
