I may have had a security breach in my website because the user was able to make an appointment using Amelia despite having 2 custom fields that were set as required. Please assist.

If a custom field is set as a mandatory field, it can't be skipped. The issue cannot be replicated on your website either:
Even if I try adding a space, to try and full the form, it's not allowing me to book.
Adding only one letter, or a symbol does allow me to move forward (see "Insurance company" and "Insured Member ID #" fields):
But still, it requires some input.
You can test the plugin yourself, and see if you can replicate the issue, but you won't be able to, not even from the back-end.
