The page doesn't actually scroll down, it remains in place, but the form shrinks, and stays in the same place on the page.

You need to add a custom JS code that scrolls the form into view when the button is clicked. Something like this:

</script>

You have WooCommerce enabled, so when the loader completes, WooCommerce's cart is displayed, and is in view correctly, so I don't know if the code above will work.

I booked a test appointment for August 3rd, using the mobile device, and it did scroll all the way up.

The only thing is that you need to wait for a few seconds before it does that. As shown in your video, you started scrolling up before the Congratulations screen appeared, so if you give it a few more seconds, it will scroll all the way up on its own.
