Report Builder can only add data based on variables, so if you add "MY CITY" and "MY COMPANY" it will be present only in one row.
The variables need to be added to all columns, just like you did with ${name.all}.
I don't quite understand the issue. Can you send us a URL to the page where the table (and the Report Builder button) is, and also the template for the report, so we can take a look?
