We don't charge for our appointments, but I would like to charge a fee if someone does not show up.
Is there a way to have them enter credit card info upon booking and process only if they miss the appointment? Or charge it upfront and refund if they do come?

I am sorry to disappoint you, but unfortunately, something like this is not possible with the plugin's built-in features.
Due to legal reasons, Amelia is not yet possible to hold payments, nor are you able to charge your customers without their consent. The only advice I have is to include deposit payments, so a certain amount or percentage is paid by all your customers. You can refund the ones that do show up, and still keep the money for no-shows.

From what I understood from your documentation, it is not possible to use the deposit feature if the service cost is $0. We do not want to charge for the service, just the "no shows". Is that possible?
You are correct, if the service costs $0, you would not be able to charge the deposit either.
So, like mentioned in my previous response, the only thing you can do is actually charge all customers the amount you want to charge no-shows, and then manually refund them once they arrive.
There's no other option at the moment.
