In fact, the new support platform is already live for Amelia and wpDataTables, and we encourage you to reach out to us there.
You'll always be able to reach us through a widget in the bottom right corner of each website, where you can ask questions, report issues, or simply get assistance.
While we still do not offer live support, a new advanced, AI-powered assistant, trained on our documentation, use cases, and real conversations with our team, is there to help with basic to intermediate questions in no time.
We're doing our best to make this transition smooth and hassle-free. After June 1st, this current support website will redirect you to the new "Contact Us" pages on our product sites.

I have watched the video 2x. I am unable to add a custom field that is required like First Name, Last Name Email Phone. I want to add Dog's Name and Dog's Age, either in the registration or before the checkout.

As far as I can see by the screenshot you attached, you didn't select Services or/and Events these fields should relate to. Please do it and try again:
Also, I'd like to mention, you should book appointments/events on the front-end being logged out from your WP User or by using incognito mode.

Okay, so I got this to work, here. However, when they check out. Only one custom field shows. The first one. Where is the dog's age?
Here is what it looks like, and this is correct? But see below The second custom field does not show up.
