    Hi there,


    I keep get an error when i try to customize the form; Request failed with status code 500.

    And now the form looks funny and not working properly.

    Thank you for choosing Amelia.

    Please open the Amelia -> Customization page, click on "Reset" and then on "Save". That should resolve the issue.
