If your booking form just loads, with nothing to show on front-end, please check if you have an employee, a category and a service created?
If you do, check if the service is visible on front-end.
That should help, but if it doesn't, please provide me a temporary WP-admin login for your site where this happens, so we could log in and take a look ‘from the inside’ as that’s the most efficient way to see and resolve the issue. We do not interfere with any data or anything else except for the plugin (in case that’s a production version of the site), and of course we do not provide login data to third party.

Michel, go to - configuration -> Activation settings and enable -> delete plugin tables. Now desinstall
Or install WP DB Cleaner and delete all tables from amelia. But first delete amelia.
