We bought the Formidable Forms Integration plugin but we are experiencing a lot of trouble. I would have selected Formidable Forms Integration for wpdatatables as product/category but I never received the purchase code so I can't fill it in and that field is required. So I chose to submit a ticket in pre-purchase section instead. Here is what we'd love help with:

1. When we uploaded the plugin it crashed our wp-admin and prevented all our admins from logging in. We had to deactivate it to make things work again. The error we received is attached to this message. It also says that "Formidable Forms integration for wpDataTables add-on requires wpDataTables version 3.0. Please update wpDataTables plugin to be able to use it!". We are using 2.1.6. When we try to update wpdatatables it only allows us to access version 2.1.6. We are not able to update wpdatatables to 3.0. Is 3.0 version a premium feature?

2. We never received the purchase code 

3. We never received login information for TMS store
