On the first screen of the appointment booking form the button to continue displays correctly from then on it only is in a block colour that you can not read the instructions

This is actually caused by a CSS conflict with your theme (or some other plugin). Please try adding this CSS to the page:
#am-continue-button { background-color: white !important; }That should correct the behavior.
