First, check if you have ZipArchive installed (reach out to your hosting provider). If it is installed, make sure to upgrade it to the latest version.
If that doesn't solve the issue, please provide me a temporary WP-admin login for your site where this happens, so we could log in and take a look ‘from the inside’ as that’s the most efficient way to see and resolve the issue. We do not interfere with any data or anything else except for the plugin (in case that’s a production version of the site), and of course we do not provide login data to third party.
You can write credentials here just check Private Reply so nobody can see them except us.

It's probably got something to do with the Macros from your template file. I created a new "TEST SUPPORT" report, and it is displayed on that page. You can download that template, and it will work fine.
So, you'll probably need to work on your template, because as you can see a template from our documentation works.

You cannot use the same data twice, that is correct. So, if you're including a variable for one column, you can't use it again, as the logic of the add-on doesn't allow that.
This has already been reported to our developers and they will work on it in the future. I cannot say when it will be fixed, but unfortunately for now it's a limitation of the add-on.
