Strange enough, the Amelia plugin is not functioning properly now (probably due to some updates?). When I login to wordpress as an admin and go the appointments booking page, the plugin loads perfect. If I am not logged in (as a normal user) and visit the link then the plugin does not work properly. I have attached a screenshot.

It looks like there is an issue with with SG Optimizer.
Can you please deactivate that and see if the issue is resolved. If it does solve the issue, try excluding Amelia's files from being cached by SG Optimizer and see if that helps. If it still behaves the same, can you please send us the plugin's .zip file, so we can debug it locally.
