I've added a custom field "Attachment" but i do not know how to put this attachment in the notification email. What is the placeholder for this function?
There isn't a possibility of sending attachments via email notifications in Amelia with its current features, unfortunately. We have this in our features list, you can check out if there is such feature request here and vote for it, but we can't say for now if and when it could be implemented.

Thank you but if i insert in the checkout the possibility to upload a file (with a custom field), how can i see this file? Is there a place in amelia where i can download this files?
You are welcome, Maurizio. You can see the custom fields in the back-end of Amelia, on the Amelia -> Appointments page - when you click on one appointment there, information about that appointment will be shown and the custom fields answers will be there too. When you click on some attachment there added via a custom field it should open it in another tab.
