I would like to send a different email based on if the user selected PayPal or On-Site payment methods?
Or can I show a different message on appointment confirmation screen based on what payment type they selected.

There isn't a possibility of sending different email notifications based on the payment method, or another thing for that matter, in Amelia with its current features. Email notifications are all the same - for all services, employees, payment methods, etc. and they are setup on the Amelia -> Notifications page.
There isn't a possibility to show a different message on the appointment confirmation screen neither, unfortunately.
