Am I able to put the service description on the booking view after a user selects a service? I'm fine with having to modify some code, was just curious why this isn't present.

Sorry for the delayed response.
Service descriptions are only visible in [ameliacatalog] booking view, and they cannot be included in [ameliabooking].
There's no built-in option that would allow you to add a form for your customers to fill. You could add custom fields to the service, if that helps?
