I have looked into some of the reasons why the booking form for service doesnt load.
None of them apply, everthing is set correctly.

Hello Laurie,
Please provide me a temporary WP-admin login for your site where this happens, so we could log in and take a look ‘from the inside’ as that’s the most efficient way to see and resolve the issue. We do not interfere with any data or anything else except for the plugin (in case that’s a production version of the site), and of course we do not provide login data to third party. You can write credentials here because this ticket is private and nobody can see it except us.

Sorry for the delay.
When you just get the spinning wheel, without it ever loading, it usually indicates that you're missing one of the following:
If you created these, please check if the service duration+its buffer times can fit in the employee's work hours, and if the option Show service on site in the Service modal is enabled.
As I can see, you don't have any a service added for 15 Minute Discovery.
So the service needs to be assigned to the employee, but under the work hours you can choose different services for different work hours, and you didn't add this service to any work hours. Please see the screenshots below:
