First you need to configure the default time slot step, so creating a 45-minute service is possible, so 5 minute step, 15 minute step or 45 minute step (this can be done in Amelia Settings/General Settings). Also, below it enable "Use service duration for booking a time slot".
Then, you configure your Service(s) to last 45 minutes, with no buffer times.
Go to your Employees section, select your employee and go to Work Hours tab. There, add working hours from 09:30 until the end of his shift.
All my 45 minute services have now been setup successfully using "Use service duration for booking a time slot"
In future we plan to add new services which will only last 20 minutes.
How do I do this please without affecting all existing services? What areas do I need to change as I am now unable to see 20 minutes for a service duration?

You would not be able to use any other time slot step because if you set it to be 10 or 20 minutes, you won't be able to use create 45-minute long services.
Once you lower the step, you'll be able to create 20-minute long services, and since you have "Use service duration for booking a time slot" enabled, when this service is displayed on front-end it will use the same logic.

I would like to set daily my appointments to start from 9:30 am to 10:15 am for a duration of 45 minutes and then the same again every hour.
For example all appointments are for 45 minutes as follows -
Monday 9:30am - 10:15am; 10:15am - 11:00am; 11:00 - 11:45am; and so on
Tuesday 9:30am - 10:15am; 10:15am - 11:00am; 11:00 - 11:45am; and so on
Same for the rest of the week
