I have added a couple of custom fields to my booking form. In the Customize panel I see a text field, then an attachment field, and finally another text field. How do I refer to these when sending out emails?

When you go to Notifications section, you will see "</> Show Email Placeholders" above the templates. Click on it to expand the list, and you will see something like %custom_field_1%, %custom_field_2%, and so on. When you hover over those placeholders, you will see which custom field that placeholder is for, and you'd be able to use it in your email templates.
