To remove "All" from "Default Rows Per Page" in the Display tab above the table, go ../wp-content/plugins/wpdatatables/templates/admin/table-settings/table_settings_block.inc.php and remove line 342:

Then, go to ../wp-content/plugins/wpdatatables/source/class.wpdatatable.php and modify line 3207 from this:

And it will remove "All" from "Show XX entries" above the table.
