To answer my own question - I just stumbled across the option right at the bottom of General Settings that explains how it works:
.../public_html/wp-content/uploads/amelia/files/
I also noticed that attachments are automatically deleted as soon as their corresponding appointments are deleted. That's great - it saves me from having to manually tidy that folder up!

Yes, you can set the path/location where you want the attachments to be saved in General settings, and by default (without you providing the path) they are saved in your WordPress Uploads folder.
