Which version of the plugin do you have? If it's not v2.6, please update it. (you should see Work Hours and Breaks radio boxes as you can see in my screenshot below)
Also, if you already have employees created - changing working hours and adding breaks to "Working Hours and Days Off" in Amelia settings will not do anything as employees' working hours will override that. So, you need to access your employees' profiles, and edit their working hours there.
You need to add your working hours, like you did.
Then, when you click on the + sign in the top right, you can select the Breaks checkbox, and choose the time in that work period when the break will be added:
