Customers cannot enter their information after selecting a date for the appointment. Why is this happening? It has worked perfectly until this week.

Apologies for replying late, we had couple of non-working days, Friday - Monday, due to a holiday.
I visited your website, and I can provide information in the booking process normally, please look at the attachment. The reason why you couldn't is probably because you were logged in to your WordPress. When you are logged in to WP, the information from your profile is filled in in the fields, and you can't change them. So, please try the booking process while not logged in to WordPress or from an incognito mode.
