I am adding custom fields in appointment booking form. I am using attachment option. How can I validate the attachment? i.e. if i want to accept only .jpeg or .png files how can I achieve it? How can I restrict user from uploading any other irrelevant media files? Also, what is the max. upload size that the plugin supports?

I am sorry to disappoint you, but unfortunately something like this is not possible with the plugin's built-in features.
The upload size depends on the upload_max_size value set in php.ini file on your server. Amelia doesn't have an upload size limit.
