When I select the event . and then press continue - the page does not request for further information like attendees name, email address and Custom Fields - which are all set up in the Customize Section of Amelia. If you're logged into WordPress, please try to view this from a private session (Incognito mode in chrome, or when you log out of WordPress). As you can see, I can view the form normally.
