    I am interested in using Amelia as my clinical website's booking plugin, I was just wondering whether I can customize the back-end Customer form and add fields to gather more information and if the customer page can track a specific customer's past appointments and services through my back end panel?

    Thanks for your interest.

    We have custom fields with which you can easily ask for any additional information you may need from your customers. You can find more about custom fields here

    On the backend, you can see past appointments for each customer and add notes.

    Final question, how do I add notes for the past appointments of my customers? Also, are all the information compiled onto a single menu or are they separated per appointment instance?

    To give you a clearer idea of what I am trying to accomplish, I am trying to create an appointment system for my veterinary clinic website, and I would love it if I had the past records of all of my clients who've reserved with the website so that I'd see whatever service and treatment they might have gotten from us already.

    Hi,

    Yes, I understand, on Appointment page you can filter per customer and date range, so you can see all appointments for respective client. When you enter appointment you can add note.
